No Cost Eye Exams and Glasses for Children. The deadline to apply is September 5th!
See flyer for dates and times to begin submitting paperwork to the Coral Shores Front Office. Some forms require a notarized Parent/Guardian signature. There will be a Notary available during the times posted on the flyer, and the Parent/Guardian must be present to notarize signatures.
In order to complete the Parking Decal and/or Off Campus Lunch Pass application, students will need to submit a Data Card, Health History form, notarized Medical Consent form, and fee. All forms are available to print by clicking the link above.
