Health History/Emergency Contact Form
🩹🌡🩺 Starting this year, students can only visit the clinic if a family member has signed the health history form giving consent. There is NO more implied consent. Today, your child’s homeroom teacher sent home a pink form. Please fill out the form and send it back with your child as soon as possible. Without a signed form, our nurses cannot provide any type of treatment. 🩺🌡🩹
